As an open collaborative group we offer information sharing and networking. We address the needs of our community through partnership and collaboration. Anyone who is interested in creating the best quality of life possible for Logan County residents are welcome to be a part of the Healthy Communities Partnership.
The HCP is funded in large part by the Abraham Lincoln Healthcare Foundation. The HCP is overseen by a steering committee including representatives from Abraham Lincoln Memorial Hospital, Logan County Department of Public Health, Memorial Physician Services in Lincoln and Mental Health Centers of Central Illinois. The Healthy Communities Partnership Manager leads the partnership and helps facilitate collaboration.
The work done by HCP is a result of its members and task forces. Whether it is an individual project or a long-term issue, we bring the critical players to the table to bring our resources together and turn our passions into action. The full HCP meets approximately 5 times per year. Task Forces meet according to need.
